1. Eligibility

This sweepstakes is open only to those who receive an entry form at Mountain View Funeral Home, Memorial Park and Crematory on Memorial Day weekend (May 22, 2026 to May 25, 2026) who are 18 years of age or older as of the date of entry. The sweepstakes is void where prohibited by law. Employees of Mountain View Funeral Home, Memorial Park and Crematory its affiliates, subsidiaries, advertising and promotion agencies, and suppliers, as well as each of their respective immediate family members and those living in the same household of their respective employees, are not eligible to participate in the sweepstakes. The sweepstakes is subject to all applicable federal, state, and local laws and regulations.

2. Agreement to Rules

By submitting an entry, you agree to be fully and unconditionally bound by these Rules, and You represent and warrant that You meet the eligibility requirements. In addition, you agree to accept the decisions of Mountain View Funeral Home, Memorial Park and Crematory in its sole and absolute discretion whether as to award or to disqualify any entry as final and binding as it relates to the content and administration of this sweepstakes. Mountain View Funeral Home, Memorial Park and Crematory reserves the right, in its sole and absolute discretion to cancel, terminate, modify or suspend the sweepstakes.

3. Sweepstakes Period

Entries will be distributed and accepted in person at Mountain View Funeral Home, Memorial Park and Crematory on Memorial Day weekend (May 22, 2026 to May 25, 2026). Entries must be made in person or online by 11:59 p.m. on May 31, 2026.

4. How to Enter

The sweepstakes can be entered by completing an entry form in person at Mountain View Funeral Home, Memorial Park and Crematory or online using distributed QR code. You may enter only once. You must provide the information requested. You may not enter more times than indicated by using multiple entries, email addresses, identities, or devices. Mail in entries are not available for this sweepstakes.

5. Prize

Five (5) Winners will receive one (1) gift card valued at $50. ODDS OF WINNING: 1:500. No cash or other prize substitution shall be permitted. The prize is non-transferable. Any and all prize-related expenses, including without limitation any and all federal, state, and/or local taxes, shall be the sole responsibility of winning entry (“Winner”). Acceptance of prize constitutes permission for Mountain View Funeral Home, Memorial Park and Crematory to use Winner’s name, likeness, and entry for purposes of advertising and trade without further compensation, unless prohibited by law.

6. Winner Selection and Notification

Five Winners will be selected by a random drawing on Monday, June 1, 2026. Winner will be notified by telephone immediately following the selection of each Winner. Mountain View Funeral Home, Memorial Park and Crematory shall have no liability for either Winner’s failure to receive notices due to the Winner’s provision of incorrect or otherwise non-functioning contact information. If either Winner cannot be contacted, is ineligible, fails to claim the prize within 7 days from the time award notification was delivered, or fails to timely return a completed and executed declaration and release as required, the prize may be forfeited.

7. Rights Granted by You

By entering this sweepstakes, you understand and agree that Mountain View Funeral Home, Memorial Park and Crematory, anyone acting on behalf of Mountain View Funeral Home, Memorial Park and Crematory and each of its representatives, agents, licensees, successors, and assigns, shall have the right, where permitted by law, to contact you for purposes of marketing its goods and services.

8.  Limitation of Liability

By entering, You agree to release and hold harmless and indemnify Mountain View Funeral Home, Memorial Park and Crematory, its affiliates, and each of their respective representatives, agents, employees, officers, members and directors from any liability, illness, injury, death, loss, litigation, claim, or damage that may occur, directly or indirectly, whether caused by negligence or not, from such entrant’s participation in the sweepstakes and entrant’s acceptance, possession, use, or misuse of any prize or any portion thereof.

9. Disputes

By submitting an entry, You agree that any controversy, dispute or claim arising out of or relating to this sweepstakes shall be resolved by arbitration administered by the American Arbitration Association in accordance with its Commercial Arbitration Rules, and judgment on the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof.
